Send-Offs

Handling a send-off can be a traumatic experience for a Referee. We have prepared several documents to help you work through the administrative details for send-off reporting. Read through this documentation before your next game. Then you will be aware of what information must be gathered, what game documents and player documents must be retained from the game, and what information must be recorded and submitted to the proper assigning authority.

While the samples are prepared using CYSA Send-Off Report Forms, the information required is similar for send-offs for the amateur divisions. CYSA requires the use of this form for all send-offs occuring in games played by teams affiliated with CYSA.The Send-Off Reporting document describes the steps to be followed in a send-off situation and contains references that can aid in preparing a Send-Off Report.

USSF has also issued a Memorandum (Match Reports Involving Discipline, April 7, 2008) which lists information to be considered when submitting a send-off report. Review the Memorandum, and keep a copy handy so you can quickly remind yourself of what you need to report when an incident occurs.

Blank CYSA Send-Off Report forms should be available from your League Referee Coordinator. A pdf version is also available on the Forms page. Down load the pdf form, complete the required information and submit the completed form along with the game card and the player or coach's pass to the appropriate Disciplinary Chairperson for your League.

Use the USSF Supplemental Game Report to report serious injuries or any other serious incident which might not be covered by the CYSA Send-Off Report.

Send-off reports for the Golden Gate Soccer League or the District 4 Competitive League are to be mailed along with the Player/Coach Pass and Game Report to:
